Agents deliver proactive insight from the entire longitudinal patient history, including the scanned documents, faxes

Today we’re launching Evidently Anywhere, which takes Evidently’s understanding of every patient out of Evidently’s own interface and puts it to work wherever health systems already work. Agents review the chart in the background and deliver what they find into the EHR and the tools teams use every day. And through an API and MCP layer, any tool a health system builds or buys can draw on that same real-time understanding. Both are already in production at major health systems, and Evidently Anywhere can be integrated on its own or alongside the rest of the Evidently platform.

Evidently Anywhere: a fleet of agents and an MCP layer that make a full understanding of every patient available in the EHR and the AI tools health systems depend on.
The Understanding Every Tool Needs
So much of a patient’s story is buried in the chart, trapped in scanned outside records, faxes from referring physicians and years of notes from other health systems. Health systems now run a growing set of tools for documentation, ambient capture, care management and the revenue cycle, from risk adjustment and clinical documentation integrity to coding and denials, and each one reads its own slice of the record. Clinicians and revenue cycle teams end up working from different, incomplete versions of the same patient: care delivered on one picture of the patient, and documented and billed on another.
Omni, Evidently’s foundational engine that has powered three years of clinical and financial outcomes for health systems across the country, reads the entire record, maps it to a medical knowledge graph and keeps a real-time picture of each patient’s full complexity. As new records arrive, Omni updates that picture and makes it available to everything in the health system’s ecosystem that needs it. Evidently is the only platform that reads the entire longitudinal patient history before anyone asks, keeps a real-time picture of each patient’s complexity, and puts it wherever the health system needs it. Thousands of clinicians already use that understanding every day inside Evidently. Evidently Anywhere opens new ways to put it to work: proactively, through agents, and on demand, through any tool the health system runs.
Health systems are now adding AI agents from their EHR vendor, their ambient partner, their platforms and their own teams. Each one is only as good as what it knows about the patient. Evidently Anywhere is built to give every one of them the same understanding of the whole record, whichever vendor built it.
Evidently Agents for Proactive Insight
Evidently Agents work in the background across inpatient, outpatient and emergency settings, looking for the insight and conditions often buried in hard-to-reach parts of the chart and acting on what changes as Omni’s picture of each patient updates. They deliver what they find wherever the health system needs it: into the EHR as SmartData Elements, advisories or rows in nursing flowsheets, into work queues, documentation integrity tools, team dashboards and registries, and into the other AI tools in a health system’s ecosystem. In turn, teams can focus on the patients and details that warrant attention, with the noise filtered out.
Evidently Agents are generally available starting today, for use cases from HCC suspecting, malnutrition and hospital-acquired condition accuracy to HEDIS measures, denial prevention, readmission risk reduction and referral triage. Health systems start with the one tied to their biggest priority and add more as they go. At two large health systems, for example, Evidently Agents read nearly every incoming referral against the patient’s full record and recommend where it should go, and at a major integrated delivery network, agents surface suspected HCC conditions inside the EHR workflow providers use every day. For needs beyond these, Evidently builds custom agents with health systems.
“Health systems spent a decade getting everything into the EHR. Understanding what’s in it is now the hardest challenge every health system faces, and it’s why Evidently exists. That understanding only matters where people and tools actually work. That can be inside Evidently, and it can just as easily be the advisory a physician already sees, the flowsheet a nurse already charts in, the referral queue or the ambient scribe. Evidently Anywhere puts it there,” said Kalie Dove-Maguire, MD, President and Chief Product Officer of Evidently.
An API and MCP Layer for Insight Exactly When Tools Need It
Through the Evidently Anywhere API and MCP layer, other tools draw on the same real-time understanding of each patient that powers Evidently itself, exactly when they need it. For health systems with their own engineering and AI teams, every app and agent they build can start from a full understanding of the patient instead of raw records. The health system stays in control of which teams and applications have access.
Today we’re also proud to announce Nabla as the first AI partner for Evidently Anywhere. At a leading academic medical center, Nabla partnered with the health system and Evidently, using the Evidently Anywhere API to pull Evidently’s longitudinal patient understanding into every listening session.
“Ambient AI’s next frontier is context. The most important facts about a patient are often not said in the room. They’re buried in outside records, scanned faxes and years of history. With Evidently Anywhere, every Nabla encounter starts from the patient’s full clinical story, and that’s live today at a leading academic health system,” said Laurent Landowski, Chief Product Officer of Nabla.
Built to Work With Every Agent a Health System Runs
Evidently Anywhere delivers into the EHR today, through whichever paths a health system chooses. It’s also built to serve Epic’s MCP Connectors when they open in spring 2027, so the agents health systems build in Agent Factory can draw on the same full longitudinal understanding of the chart.
Evidently Anywhere is built on our Responsible AI foundation. The health system decides which agents run, on which patients, and exactly where each result is delivered: an advisory in the EHR, a nursing flowsheet, a registry, a referral work queue, or another AI tool in its ecosystem. Results arrive as suggestions, and clinicians, coders and documentation specialists decide what becomes part of the documentation or the claim. Evidently partners with each health system’s AI governance team to ensure every deployment meets its standards for safe, responsible use, with observability into what each agent does and delivers.
Omni now powers Evidently Anywhere’s agents and API, with 97% suggestion accuracy in a peer-reviewed study and four to five quality-impacting conditions found missing per admit note across five health systems.
About Evidently
Evidently is the leading Clinical Data Intelligence platform, bringing the entire longitudinal patient history back into the EHR workflows clinicians and revenue teams rely on every day, with every insight linked back to its source. That includes the scanned documents, faxes, outside records, imaging reports, and data in external systems like Care Everywhere and CommonWell that the EHR was never built to read. Allina Health, UNC Health, University of Iowa Health Care, and other leading health systems across the United States use Evidently across inpatient, outpatient, and emergency settings, with peer-reviewed accuracy and published clinical and financial outcomes.
